Parks and Recreation Forms (Community Services Department)

Here you will find the various forms needed for the recreational programs run by the City of Naples Parks and Recreation division.  These forms are interactive, meaning you can fill them out in your browser and print or email the forms as needed.  Some forms require payments which cannot currently be made online.  Others may also require in-person delivery.  Please check each form for instructions.

There are a few requirements for using these forms:
 

  1. Adobe Reader or Adobe Acrobat (Windows of Mac), preferred version 8.1 or higher. Mac users should take extra caution not to use to Apple “Preview” which is Mac’s default PDF viewer. For proper rendering, all
  2. Forms must be opened in Adobe Reader/Acrobat (other third-party PDF viewers should not be used – i.e. Nitro PDF, Foxit Reader, etc).
  3. If saving entire PDF form with data is desired, users should complete the form using free Adobe Reader version 11 (or greater), or any version Acrobat Std/Pro.
  4. If forms will be placed on a website for user access, it is highly recommended that users use Internet Explorer (IE) or Opera web browsers to access the forms. IE and Opera browsers typically use the Adobe Reader plug-in to render PDF documents within the browser. Chrome and FireFox 19+ both use custom plug-ins to render/display PDF documents (which is typically OK), but can cause issues when rendering “PDF forms”. Depending on the version and/or setup of Apple Safari, it too may be used.  In any such case, users should ensure the Adobe Reader or Acrobat application is installed on their local machine which also installs the necessary web browser plug-in.
  5. Mobile device users can use Adobe’s free Mobile Reader app for iOS (iPhone/iPad) and/or Android.
